Page 1: Project information and consent

Does your child struggle accessing school?  We are investigating the impact of SEND (Special Educational Needs and Disabilities) on a child’s wellbeing and learning.  We're keen to hear the opinions and experiences of parents/carers. 

All questions are optional, so please just answer the questions relevant to you and your child.

Please be assured that your responses will be recieved in an open-minded and non judgmental way so please feel free to be honest.  All questions are optional so feel free to only answer the ones which are relevant to you.  All responses are anonymous and we will not write anything up which would be identifiable of you.

You will be contributing to the field of knowledge in this area, and also raising awareness of the challenges faced by children with undiagnosed/suspected SEND.

There is minimal risk in taking part as the survey will be completed online, so you can do this in your own setting in your own time.

The researchers are Gill Rogerson and Sarah Holmes at Liverpool Hope University.  Please email them at holmess1@hope.ac.uk if you have any queries or if you would like to withdraw.  (You can withdraw up to two weeks after you've completed the survey.)

Please note that your confidentiality and anonymity cannot be assured if, during the research, it comes to light that you are involved in illegal or harmful behaviours that I may need to disclose to the appropriate authorities.
